Pro: Forms & Waivers is a $10/month add-on. It is optional. OperMae does not charge it until you add it. On an annual Pro plan it is still $10/month, with no annual discount.
Team: Forms & Waivers is included. Team is not asked to buy it.
Where
On Pro, open Add-ons and choose Add Forms & Waivers — $10/month. After it is active, open Settings → Forms & Waivers.
What you can create
A form or waiver can include questions, required fields, a checkbox acknowledgement, and a signature acknowledgement. Assign it to services, then publish it. You can edit, deactivate, or duplicate it later.
If you cancel
Canceling the add-on removes Forms & Waivers from the next paid period. Saved forms and submissions stay in your account. They are not deleted.
Signature acknowledgement
A signature acknowledgement records that the client acknowledged the text. It is not described as a legally binding electronic signature.
